Please arrive 10-15 minutes prior to your appointment to fill out a health history/ intake / contact form. 15 minute consultations are available by appointment at no charge.

                                                                                            Payment Methods

• Payment is expected at time of service.
• Cash and checks are preferred method of payment.A convenience fee is applied towards all debit and credit purchases.
  
• A $35 service fee will be charged for all returned checks.
• Gift Certificates are available. Gift certificates are non-redeemable for cash. Gift certificate recipient is responsible for amount due (should price increase) after expiration, for redemption. At this time all In Sync Mind Body Therapy gift certificates may be used towards services only.Gift certificates are available for services only, and not redeemable for products.

• To accommodate and simplify life please feel free to schedule your next appointment upon leaving.
• 24 hour advance notice is greatly appreciated when canceling or rescheduling appointments.This allows the opportunity for someone else to schedule an appointment.
• If you are unable to give 24 hour advanced notice to cancellation or rescheduling, you can be charged for the full amount of your service. This amount must be paid prior to scheduling your next appointment.

Appointment times have been arranged specifically for you. Depending on your service you may be asked to reschedule with full fee of service, or the length of your service will be shortened with full fee.
